3.本项目的特定资格要求：





合同包1(2022年整村推进无害化卫生厕所改造及问题户厕维修项目)特定资格要求如下:





（1）提供合格有效的法人或者其他组织的营业执照等证明文件，自然人的身份证明；

（2）法定代表人授权书（附法定代表人、被授权人身份证复印件）及被授权人身份证（法定代表人直接参加招标，须提供法定代表人身份证明及身份证复印件）；

（3）提供供应商2021年全年度经审计的财务报告，或者提供开标前1个月内由开户银行出具的资信证明；

（4）提供投标人2021年5月至开标前任意三个月已缴纳的完税证明或纳税凭证；

（5）提供投标人2021年5月至开标前任意三个月已缴纳的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明；依法不需要缴纳社会保障资金的应提供相关文件证明；

（6）提供投标人具备履行合同所必需的设备和专业技术能力的声明；

（7）投标人参加本次招标前3年内，在经营活动中没有重大违法记录的书面声明；（8）根据财政部《关于在政府采购活动中查询及使用信用记录有关问题的通知》（财库〔2016〕125号）要求，供应商不得被列入【信用中国】“失信被执行人”、“重大税收违法案件当事人名单”，【】“政府采购严重违法失信行为记录名单”，栏目中有失信等负面信息的供应商，将拒绝其参加本项目（供应商无需提供相关信息查询截图，在资格审查时通过互联网或者相关信息系统查询）；

（8）保证金缴纳凭证或担保机构出具的保函;

（10）本项目不接受联合体投标。

注：单位负责人为同一人或者存在直接控股、管理关系的不同投标人，不得参加同一合同项下的政府采购活动。
