3.本项目的特定资格要求：





合同包1(电子喉镜)特定资格要求如下:





（1）对于国外、境外投标人，根据该国（地区）的法律在经营所在地注册的有关证件；

（2）对于国内投标人，提供投标人的企业法人营业执照、投标人的医疗器械经营企业许可证（代理商作为投标人适用）、制造商的医疗器械生产企业许可证（国外制造商除外）（如产品具有医疗器械注册证须同时具备）；

（3）拟参加本项目的投标人应具备《中华人民共和国政府采购法》第二十二条规定的资格条件：

（4）拟参加本项目的投标人须在黑龙江省政府采购网上注册登记并备案合格；

（5）拟参加本项目的投标人所投进口产品须具有合法证明文件；

（6）拟参加本项目投标的潜在供应商、供应商法定代表人未被列入重大税收违法案件当事人名单，核查路径：<信用中国>查询网址：
